DOR Director of Rehab

Job Summary:

Dahlia Gardens Center for Nursing and Rehabilitation, a member of the Alliance Health Group family of skilled nursing facilities and rehabilitation centers, is seeking a dynamic, experienced, and compassionate DOR Director of Rehab to lead our skilled therapy department. This is an exciting opportunity for a motivated rehabilitation professional to make a meaningful impact in the lives of our residents while leading a dedicated interdisciplinary team in a supportive and collaborative environment.

Essential Responsibilities:

  • Lead and oversee the daily operations of the rehabilitation department

  • Supervise, mentor, and support a multidisciplinary team of therapists and therapy assistants

  • Collaborate closely with nursing, physicians, case management, and administration to ensure coordinated resident care

  • Ensure timely and accurate documentation in compliance with regulatory, clinical, and billing standards

  • Develop, implement, and monitor individualized treatment plans focused on resident goals and outcomes

  • Monitor departmental performance, quality measures, and compliance initiatives

  • Provide ongoing staff education, coaching, and performance evaluations to support professional growth

  • Maintain compliance with all federal, state, and facility policies and procedures

  • Serve as a key liaison for residents, families, physicians, and healthcare partners regarding rehabilitation progress and care planning

  • Foster a positive, respectful, and team-oriented culture throughout the department and facility

Qualifications:

  • Current license as a Physical Therapist (PT), Occupational Therapist (OT), or Speech-Language Pathologist (SLP) in the state of North Carolina (required)

  • Minimum of 3 years of clinical experience in a skilled nursing or long-term care setting

  • At least 1 year of rehabilitation leadership or management experience preferred

  • Strong knowledge of Medicare regulations, MDS processes, and reimbursement practices

  • Excellent leadership, communication, organizational, and interpersonal skills

  • Passion for delivering exceptional resident-centered care and building strong clinical teams
