JOB SUMMARY:

Working as a Caregiver in our Community Based Care Facility, you will assist residents with daily activities, promoting independence, and ensuring their safety and well-being, while also providing companionship. Caregivers work closely with residents, their families, and care team to provide personalized care that meets each individual's unique needs and preferences.   


RESPONSIBILITIES:
  • Provide assistance with activities of daily living (ADLs).
  • Monitor and report any changes in residents physical or mental health.
  • Assists with serving resident meals.
  • Provide companionship and build positive relationships with clients to reduce feelings of isolation or loneliness.
  • Assist in maintaining accurate records of care provided, including daily logs, progress notes, and incident reports.
  • A Caregiver will maintain resident confidentiality.
  • Encourage clients to participate in activities that promote physical, mental, and emotional well-being.
  • Support clients in maintaining independence in their daily routines and decision-making.
  • Ensure a safe environment by identifying and addressing potential hazards in community settings.
  • Respond promptly and appropriately to emergencies, such as falls, injuries, or sudden health changes, following established protocols.
  • Collaborate with the care team, including healthcare professionals, social workers, and family members, to provide coordinated care.
  • Communicate effectively with residents, families, and team members to ensure clarity and understanding of care plans and any changes.
  • Present for staff meetings and training.
  • Other duties as assigned.


REQUIREMENTS:
  • Must be 18 years of age
  • High school diploma or equivalent required.
  • Previous experience in caregiving, home care, or community-based care is preferred.
  • Ability to work independently and as part of a team.
  • Patience, empathy, and a genuine desire to help others.
  • Good organizational and time management skills.
  • Ability to perform physical tasks such as lifting, bending, and assisting residents with mobility.
  • Must be able to stand, walk, and reach for extended periods.
  • Ability to pass a background check.
