Job description

A Self Direction Coach is responsible to implement the services and supports that have been created in the individualized Community Habilitation plan in conjunction with the planning team members.

Upstate Caring Partners is looking for energetic and motivated staff to work with individuals with disabilities to help them achieve their goals!
  • Work directly with individuals with developmental disabilities to plan and also provide habilitative (skill building) and supports as determined by the Self-Direction budget and the individuals person centered support team.
  • Work to increase skill level and decrease challenging behaviors of individuals with developmental disabilities by addressing goals outlined in their individual self-direction plan.
  • Provide services within the person's home and in their community in order to promote independence, inclusion and quality of life.

QUALIFICATIONS
  • High School Diploma or GED required. Associates Degree in field of psychology or related field preferred.
  • Must have understanding of the characteristics of persons with developmental disabilities and behavior management.
  • At least 1-year working with people with developmental disabilities and behavior management is preferred.
  • Valid NYS Driver's License required.
  • No previous experience needed - we provide paid training!
  • Starting pay $16.00/hour for Agency Training (Pay is based on family budget)
