Job Location: Division of Developmental Disabilities Specialty District Serving Districts Central, East and West 4000 North Central Avenue, Phoenix, Arizona 85012

Salary: $81,000- $85,000

Grade: 22

The Department of Economic Security, Division of Developmental Disabilities (DDD), is seeking an experienced and highly motivated individual to join our team as a Registered Nurse Case Manager in the Specialty District. DDD provides individuals with developmental disabilities, and their families, services and supports that are flexible, high quality, and member driven. These services provide individuals with opportunities to exercise their rights and responsibilities of independent decision-making and engagement in the community.

Job Duties: Essential Duties and Responsibilities include but are not limited to:

  • Convene and facilitate in-person, multi-disciplinary Person-Centered Service Plan (PCSP) meetings to determine and identify services. Coordinate and monitor member long term care, physical and behavioral health services. Responsibilities include vendor calls for long term care services, CRS referrals, specialized transportation, discharge, specialty care, arranging for medical equipment, coordinating skilled nursing or home health services.
  • Completes additional documentation pertaining to care coordination of physical and behavioral health needs of Division members. Attends staffing and discharge planning meetings and talks/meets with appropriate health plan and health care professionals to network to provide continuity of care for member.
  • Conducts assessment of member's physical, behavioral, & long-term care needs to develop comprehensive care plans for prevention, outcome improvement, decrease risk status, & increase self-care opportunities. The assessment to include Social Determinants of Health; social & economic factors, natural & built environments & policies & programs. Write comprehensive progress notes and maintain case records.
  • Conducts on-site visits to member/family homes, schools, and program sites to discuss member's progress, needs and develop relationships. Become familiar with family dynamics/needs/supports and make appropriate referrals to community agencies. Provides oversight relating to services the member receives and reports quality of care concerns promptly per DDD's policies and procedures. Assists with members/families in crisis.
  • Assesses nutritional status and needs of member and makes referral to RBHA, Dietician, Primary Care Physician, or health plan, as appropriate. Assists in assigned areas to complete audits/fact finding reviews; monitors and completes cost effective studies on assigned members. Prepare a variety of reports to include a Licensed Health Aid Hourly Nursing Assessment Tool.

Knowledge, Skills & Abilities (KSAs):

  • Knowledge in: Managed Care, Medicaid & Medicare concepts Case management concepts, principles, and strategies Nursing theories and practices related to individuals with developmental disabilities and behavioral health conditions Interdisciplinary team planning processes Person centered planning strategies
  • Skills In: Case Management (Interviewing, Advocacy, Collaboration) Comprehensive nursing assessment & needs analysis Building consensus, continuous quality improvement and financial accountability Critical Interviewing techniques Developing, analyzing and implementing health treatment plans
  • Ability to: Work with other physicians and health professionals Work with the legal system Observe and assess health status and needs of individuals with developmental disabilities and behavioral health conditions Prioritize and multi-task to manage workload and meet time lines Interpret and apply DDD Policies and Procedures

Selective Preference(s): The ideal candidate for this position will have: A Bachelor of Science in Nursing (BSN) from an accredited college or university. Two years of experience working as a registered nurse in behavioral health, home care, hospice or long-term care is highly desirable. Knowledge of the current Medicaid and Behavioral Health service delivery system a plus.

Pre-Employment Requirements:

  • Required at time of appointment: possession of a current license to practice as a registered nurse in the State of Arizona.
  • Employee shall have a valid Level One fingerprint clearance card issued pursuant to Arizona Revised Statute 41-1968 in order to work with children and vulnerable adults.
  • Candidates for this position shall be subject to a search of both the Department of Child Safety Central Registry pursuant to A.R.S. 8-804 and the Adult Protective Services Registry pursuant to A.R.S. 46-459.
  • Successfully pass background and reference check; employment is contingent upon completion of the above-mentioned process and the Agency's ability to reasonably accommodate any restrictions.
