Job Summary

Position title: Clinical Assistant, Business Development(Nursing Home)

Job summary: The Business Development Clinical Assistant will support the Director of Business Development in driving key clinical and operational initiatives

across our partner nursing home facilities. This role involves following up with sites that have already been visited by the Director, ensuring

continued implementation of programs such as APCM, BHI, and TOC. BD CAs will act as liaisons between the corporate team and onsite staff,

helping to maintain momentum on business development goals and provider engagement. This position requires strong communication,

organizational, and follow-through skills.

Responsibilities
  • Follow up with nursing homes previously visited by the Director of Business Development to support ongoing
  • implementation efforts.
  • Obtain APCM and BHI consents from patients and/or facility staff for new and existing admissions.
  • Reinforce TOC (Transitions of Care) referral workflows at the time of patient discharge.
  • Serve as an onsite resource to providers and Clinical Assistants, answering questions and clarifying processes
  • related to corporate initiatives.
  • Track and report progress on facility compliance with APCM, BHI, TOC, and other strategic programs.
  • Identify barriers to implementation and escalate issues as needed to the Director of Business Development.
  • Assist in scheduling follow-up visits and maintaining communication between corporate and facility teams.
  • Facilitate implementation of additional corporate initiatives as needed.
Qualifications
  • Backelors Degree in related field
  • Excellent communication skills 
  • Confidence when speaking to clinicians and business leaders 
  • Computer literate (Excel, Word, Outlook, Teams) 
  • Strong organizational skills to give team direction 
  • Knowledge of care provided in Nursing Homes 
  • Aware of and complies with E&M Guidelines for Nursing Home Care 
  • Able to navigate various EHR systems
