COMMUNITY COACH ~Designated Exceptional Services for Independence (DESI) Make a Difference. Change Lives. Build Independence.

DESI is seeking compassionate, reliable, and motivated individuals to join our team as Community Coaches. This rewarding position provides one-on-one support to young adults with developmental disabilities as they learn life skills, build confidence, and become more independent members of their communities.

Service Areas: East Los Angeles, Southeast Los Angeles County, and surrounding communities

Flexible Schedule – Full-Time & Part-Time Opportunities Available

What You'll Do:Key Responsibilities:
✅ Provide one-on-one coaching and support in community settings-

✅Maintain thorough documentation of consumer progress, including daily progress notes.

✅Attend bi-monthly team meetings and weekly check-ins with supervisors.
✅Collaborate with the Case Manager to coordinate and plan daily schedules.
✅Serve as a liaison between consumers, families, and external agencies.
✅ Provide 1:1 support to consumers, fostering positive and empowering learning experiences

✅ Assist individuals with developing independent living skills
✅ Support participation in community activities and social events
✅ Help clients achieve personal goals through person-centered planning

✅ Daily activities that align with consumer goals and objectives.
✅ Teach skills such as budgeting, transportation, communication, self-advocacy, and daily living tasks
✅ Document progress and maintain professional records

Qualifications:

✔ High School Diploma or GED required
✔ Valid California Driver's License and reliable transportation preferred but not needed
✔ Strong communication and organizational skills
✔ Ability to work independently and as part of a team
✔ Experience working with individuals with developmental disabilities is preferred but not required
✔ Bilingual English/Spanish is a plus
