Key Responsibilities
  1. Conduct and coordinate comprehensive assessments of new and existing residents using the MDS tool.
  2. Develop, implement, and review individualized care plans in collaboration with an interdisciplinary team.
  3. Ensure compliance with federal and state regulations for MDS assessments and care planning.
  4. Monitor and transmit MDS assessments to the relevant authorities.
  5. Train and guide nursing staff on MDS procedures and updates.
  6. Audit clinical records to ensure accuracy, completeness, and regulatory compliance.

Qualifications
  1. Active RN license required.
