PRN Occupational Therapist (OT) – Home Health

Deer Meadows Home Health & Support Services

Deer Meadows Home Health & Support Services is seeking a compassionate and dedicated PRN Occupational Therapist (OT) to join our growing home health team. This is an excellent opportunity for an OT who enjoys working independently while making a meaningful impact by helping patients achieve greater independence in the comfort of their own homes.

Responsibilities
  • Evaluate patients and develop individualized occupational therapy treatment plans.

  • Provide skilled occupational therapy services in the home setting.

  • Assess functional abilities, home safety, adaptive equipment, and activities of daily living (ADLs).

  • Educate patients, families, and caregivers on treatment plans and home safety recommendations.

  • Collaborate with physicians, nurses, and the interdisciplinary care team to ensure optimal patient outcomes.

  • Complete accurate, timely documentation using Homebase Home Care EMR.

  • Maintain compliance with agency policies, Medicare guidelines, and state and federal regulations.

Qualifications
  • Current Occupational Therapist (OT) license in PA.

  • Graduate of an accredited Occupational Therapy program.

  • Home health experience preferred, but not required.

  • Experience with Homebase Home Care EMR is a plus.

  • Strong clinical assessment, communication, and organizational skills.

  • Current CPR certification (or ability to obtain).

  • Valid driver's license, reliable transportation, and current auto insurance.
