JPNDC's Family Childcare program (FCC) is a system of 115+ independent licensed early educators offering childcare in their homes to low-income children with public childcare subsidies. JPNDC supports educators in providing high-quality early education and care designed to prepare children for success in school and life. We also support families through an array of wraparound services and enrichment activities.

The Role: The Case Manager will join the Community Wellness Team for Family Childcare (FCC), reporting directly to the Sr. Case Manager for Community Wellness. As a dedicated and skilled professional, the Case Manager will serve as the family liaison for our priority populations, including DCF, young parents under 24, and families experiencing homelessness. In collaboration with various agency partners, the Case Manager will support individualized family needs, facilitate tailored Family Plans, and conduct developmental evaluations. All program staff members supporting FCC are considered mandated reporters and will be supported with the requisite training as needed. This role is ideal for someone passionate about making a positive impact on the community by supporting families and children, particularly those with high needs facing challenging circumstances.

Duties:

  • Liaison for our priority population such as: DCF, Young Parents Under 24, and Families Experiencing Homelessness.
  • Participate in regular meetings with DCF, Shelter Systems or other family service agencies.
  • Support contractual needs of the children and families including referrals to community resources and multi-agency reporting as required.
  • Facilitate and track cross department referrals with JPNDC Economic Prosperity Services to develop Individualized Family Plans.
  • Perform developmental evaluations such as: ASQ's/ASQ-SE's for newly enrolled and families as needed.
  • Facilitate and supports parent workshops, education, self-sufficiency, and other related enrichment activities for priority population.
  • Provide families with ongoing support in navigating referral processes such as: Early Intervention, Public School Systems, Public Assistance Benefits, and/or Hospitals.
  • Provide one-on-one assistance and referrals to supervisor to help identify high-needs families.
  • Conduct routine home visits to educator programs and prepare related reports.
  • Other duties as assigned by supervisor.

Qualifications:
  • Required:
    • DECC Lead Teacher -Infant Toddler/Preschool certified
    • Bachelor's degree in Social Work or a related Human Services Degree.
    • Minimum 1 year of case management/counseling experience with children, adolescents, and families in DCF or shelter settings, plus strong knowledge of Early Childhood Education, Social Work, or related Human Service field and community resources
    • Proven experience serving multicultural, underprivileged populations and trauma-affected families, with strong cultural competency and ability to handle confidential information professionally
    • Fluent in English and at least one additional language: Spanish, Haitian Creole, or Cape Verdean Creole (written and verbal proficiency required)
    • First Aid and CPR certified (certification training available).
    • Ability to work a flexible schedule with occasional evening and weekend availability
    • Valid driver's license and reliable personal vehicle required for daily work-related travel
  • Preferred
    • In the process of course credits toward LSCW and/or LSW
  • Key Skills:

    • Organized, strong attention to detail and solution oriented.
    • Ability to work flexible schedule consistent with program hours of operations/business needs.
    • Solid oral and written communication skills.

Employment Terms/Compensation: This is a full-time, salaried position with a generous benefit package. Salary range is $60,000 - $75,000.
