Job description

Summary

The HB Coding Analyst II is responsible for accurate review, abstraction, coding, indexing, and charge capture of hospital-based services, including outpatient, inpatient, and the computation of observation time charges, using ICD-10-CM, ICD-10-PCS, CPT/HCPCS, DRG, and APR-DRG coding systems as applicable. This role supports regulatory compliance, revenue integrity, accurate reimbursement, and timely claim submission.

Key Responsibilities

  • Reviews and codes all diagnoses according to ICD-10-CM coding classification systems and regulatory guidelines. Maintaining a 95% or higher quality rating.

  • Reviews and codes all procedures according to ICD-10-PCS coding classification systems and regulatory guidelines. Maintaining a 95% or higher quality rating.

  • Reviews and codes all procedures according to CPT coding classification systems and regulatory guidelines. Maintaining a 95% or higher quality rating.

  • Reviews and codes all accounts according to DRG & APR-DRG coding classification systems and regulatory guidelines. Maintaining a 95% or higher quality rating.

  • Maintain 95% or higher on all monthly coding educational knowledge checks.

  • Demonstrates a consistent level of performance; strives to maintain a minimum productivity that is set per chart type.

  • Abstracts and indexes medical records, according to governmental compliance and hospital guidelines.

  • Query physicians for clarification of conflicting/dropped documentation.

  • Review cardiac charges, remove 481 code, and post appropriate charges.

  • Work with leadership in troubleshooting third party coding software.

  • Attend all CDI/Coder/Case Management monthly meetings.

  • Supports and participates in meeting departmental goals.

  • Displays professionalism and courtesy in assisting physicians and other departments in problem solving.

  • Reviews outpatient records, computes observation time, and posts the charges to the patient account.

  • Performs all other tasks/responsibilities as necessary/assigned.

Qualifications 

  • High school diploma or equivalent required

  • Coding or related Certification or Vocational education in field preferred

  • Strong knowledge of:

    • Medical terminology and anatomy

    • Coding guidelines

    • ICD-10-CM diagnosis coding

    • ICD-10-PCS procedure coding

    • CPT/HCPCS procedure coding

    • DRG and APR-DRG coding systems

  • Strong attention to detail and organizational skills

  • Must be able to communicate effectively in English, both verbally and in writing

  • Ability to work independently and as part of a team

Experience 

  • Minimum of one-year experience inpatient and/or outpatient coding in an acute health care setting.

Physical Requirements 

  • Vision acuity, hearing sensitivity, and manual dexterity
