Job description

Coastal Hospice invites you to join a team of caregivers who are committed to promoting dignity and quality of life for patients and families who face life-limiting conditions.  In your role as an _RN at Coastal Hospice by the Lake____, you will help ensure that every patient and family is honored, supported and comforted as they receive the right care at the right time throughout their journey.  You will:

• Responsible for developing, implementing and evaluating a plan of patient care utilizing ANA (American Nurses Association) Standards and the Nursing Process

• Utilizes equipment and technology skillfully and is technically competent in nursing procedures

* Oversees and/or participates in safe medication handling and practices

• Educates patients and families on significant self-care and end of life issues, all hospice services, patient liability for payment, and ensures consent and payer forms are completed

• Acts as a patient advocate; provides support, information and direct care

• Triages phone calls to identify patient/family needs and notifies Admission Services of referrals

• Oversees and delegates assignments of CMTs, LPNs, Hospice Aides, students, and volunteers on shift

• Participates in weekend, holiday, on-call and/or evening/night rotations

• Maintains appropriate licenses and adheres to continuing education obligations

• Ability to cover different areas of hospice care to assure organizational needs are met

Inpatient Unit RN:

• Provides medical care to hospice patients in an inpatient setting

• Completes assessments, administers medications, and manages plan of care for patients during their shift

• Coordinates discharge planning with family and medical provider

Education and Work Experience Requirements:

• Licensed and currently registered to practice as a Registered Nurse in the State of Maryland, Delaware or other compact state
• BSN from an accredited college preferred
• CHPN certification expected within two (2) years of employment
