Carolina Caring is looking for a dedicated and compassionate Master of Social Work (MSW) to join our team, providing vital case management and counseling services to patients, families, and caregivers during critical moments.
As a Medical Social Worker, you will perform in-depth psychosocial assessments and develop individualized care plans in collaboration with our Interdisciplinary Team, ensuring holistic and supportive care for every patient.
This role will focus on serving home hospice patients in Davie County.
This full-time position offers a Monday-Friday schedule, 8:30 a.m.-5:00 p.m., along with a competitive benefits package, paid time off, and mileage reimbursement.
If you are passionate about making a difference in hospice care, this is an incredible opportunity to support and guide patients and families through their journey with empathy and expertise.
Requirements
Education: Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education.
