Join Synergy’s dynamic team as a Data Integrity Specialist focused on ensuring clean, accurate, and reliable physician and facility data within Homecare Homebase (HCHB). We are seeking a detail driven, systems minded professional to serve as the steward of master data that directly impacts orders, certifications, billing, compliance, and clinical workflows across Home Health and Hospice. This role plays a critical operational function by maintaining a single source of truth and ensuring downstream processes are supported by high quality data.

About the Role

The Data Integrity Specialist is responsible for owning and managing physician and facility master data within Homecare Homebase. This role serves as the point of accountability for data accuracy, standardization, and governance, ensuring all additions, updates, and changes meet established quality and compliance standards. Working closely with clinical leadership, operations, intake, billing, and IT partners, this position protects the integrity of core system data and prevents avoidable errors that impact patient care and reimbursement.

Key Responsibilities

Master Data Ownership & Stewardship
• Serve as the primary steward for physician and facility master data within Homecare Homebase.
• Own data standards, naming conventions, and required data elements to ensure consistency across agencies.
• Review, approve, and manage all requests for new or updated physician and facility records.
• Ensure master data remains accurate, current, and aligned with operational and regulatory requirements.

Data Quality & Integrity Management
• Proactively identify, investigate, and resolve duplicate, incomplete, or inaccurate records.
• Conduct routine audits to ensure physician credentials, identifiers, addresses, and affiliations are correct.
• Monitor downstream impacts of data errors on orders, certifications, billing, claims, and reporting.
• Maintain documentation and controls to support ongoing data integrity and audit readiness.

Cross Functional Collaboration
• Partner with clinical, intake, billing, compliance, and agency leadership to resolve data related issues.
• Serve as a subject matter expert for physician and facility data usage within HCHB workflows.
• Provide guidance on best practices for requesting and maintaining master data.
• Support onboarding of new agencies or providers by ensuring clean data setup from the start.

Governance, Compliance & Risk Mitigation
• Ensure master data aligns with payer requirements, regulatory standards, and internal policy.
• Identify trends or systemic issues that increase compliance, billing, or operational risk.
• Escalate high risk data issues and recommend corrective actions to leadership.
• Support internal audits, external reviews, and compliance initiatives as assigned.

Process Improvement & Documentation
• Develop and maintain standard workflows for master data requests, approvals, and changes.
• Recommend system or process improvements to reduce manual rework and prevent recurring issues.
• Create clear documentation and job aids to support consistent data practices across teams.
• Perform other duties and special projects as assigned.

Qualifications

Required
• Strong understanding of master data concepts and data quality principles.
• Experience working with healthcare information systems; Homecare Homebase (HCHB) experience strongly preferred.
• Knowledge of physician and facility data requirements within home health and hospice operations.
• Ability to work independently while collaborating effectively with interdisciplinary teams.
• Excellent attention to detail and analytical problem solving skills.
• Strong written and verbal communication skills.
• Proficiency with Microsoft Outlook and data driven systems or reporting tools.

Preferred
• Experience in home health or hospice operations, intake, billing, compliance, or clinical support.
• Familiarity with downstream impacts of data integrity on orders, certifications, and reimbursement.
• Prior experience in data governance, data management, or system administration roles.
