Position Summary

We are seeking a detail-oriented and client-focused Intake Coordinator to manage the admission and discharge process for individuals entering and completing care at our facility.

As the first point of contact, the Intake Coordinator ensures all required documentation, assessments, and communication are completed accurately and efficiently-helping create a smooth and supportive experience for both clients and staff.


Key Responsibilities
  • Coordinate and complete admission paperwork, collect documentation (e.g., photo ID, insurance), and collaborate with nursing for client onboarding
  • Conduct level-of-care assessments and complete PCPC or ASAM documentation for admissions, continued stays, and discharges
  • Manage insurance-related processes, including precertifications and utilization review (UR) notes
  • Assist SCA clients with the Medical Assistance application process, including communication with representatives and submission of required forms
  • Maintain communication with outside stakeholders (e.g., probation/parole officers, employers) with proper client consent
  • Organize and distribute client photos, label with MR numbers, and file appropriately
  • Support discharge processes for detox-only clients, including scheduling outpatient appointments and coordinating with clinical and nursing teams
  • Conduct phone and in-person pre-screens, enter data into the electronic clinical record, and schedule assessments or referrals
  • Ensure compliance with confidentiality, licensing, and accreditation standards
  • Complete required trainings and perform other duties as assigned

Qualifications

Education:

  • High school diploma or equivalent required

Experience:

  • Experience working with individuals in behavioral health or substance use treatment preferred

Skills:

  • Strong communication, organizational, and attention-to-detail skills
  • Ability to work collaboratively and compassionately in a fast-paced environment
