We are looking for an Clinical Liaison!

The Clinical Liaison has an essential role in ensuring a seamless transition for all admissions from acute or post acute care. Your collaborative engagement is significant to coordinate admissions and communication to maintain continuity, and ensure the best care, our communities have come to expect. You will generate and manage referrals and admissions from hospitals. You will take lead developing relationships with hospital discharge planners, and developing initiatives that align with market to grow business and identify new opportunities. Liaisons work very closely with our AD Concierge and leadership, to drive daily admissions. Time Management, and multi- tasking will be necessary for market growth. Our staff and residents will rely on your knowledge and detail to achieve the best patient experience.

Role Responsibilities

  • Stationed daily at primary account to maximize market engagement, and referral review efforts.

  • Review and update CRM Dashboard for accurate reporting via Forecast and other communication platforms.

  • Respond to all incoming referrals on Careport and/or other referral platforms timely, daily, and throughout the day.

  • Provide definitive answers for all incoming referrals within an hour of receiving and/or by end of day, if appropriate.

  • Communicate/Collaborate with Business Development and Operations leadership to expedite any delays, barriers, and solutions in real time to expedite, definitive answers.

  • Timely PCC data input including patient demographics, resident representative, POA, and Guardian information.

  • Weekly Reports

  • Participate in Quarterly Business Development meeting, aligning initiatives grow census, and market exposure.

  • Complete daily Forecast for Team review via Census call, Morning meetings and/or standdown.

  • Overseeing Guest Relations and Concierge initiatives including patient rounding, assisting with testimonials, and promoting positive outcomes.

Qualifications

  • Must possess, as a minimum, a GED, or High School Diploma, with 2 years’ experience. Preferred associate degree in healthcare, business or similar with 1 year experience

  • Must meet health assessment requirements regulated by the state.

  • Must be capable of performing the essential functions of the job.

  • Must have initiative, patience, compassion, self-motivation, and solution oriented.
