Job description

Within the Stead Family Department of Pediatrics, Division of Pulmonary Medicine, this position evaluates center outcomes, addresses quality improvement initiatives and examines dynamic health care issues, including nutritional status, infection control and pulmonary treatment and/or metabolic issues rapidly and effectively for cystic fibrosis (CF) patients. Responsibilities would also include data entry to keep the Patient Registry current and accurate.
The Nurse Coordinator will be primarily dedicated to the CF Program. However, occasional cross-coverage for other nursing/clinic needs may be required to support a collaborative team environment. These cross-coverage needs will be discussed in advance with the CF program director and nursing supervisor to ensure proper coordination and alignment.
This position requires a Baccalaureate degree in Nursing.
Duties to include, but are not limited to:
Compliance with Internal Policies and External Regulations for Safe and Effective Care Delivery and Health Profession Education
CF Patient Registry PORTCF
  • Assist the primary investigator (PI)/Center director in overseeing mechanisms to ensure accurate data entry into CF Patient Registry
  • Review CF Patient Registry data entry fields to understand information to be collected and work with hospital systems to ensure data collection
  • Periodically review data collection forms and processes for changes
  • May oversee and/or perform data entry depending on staffing and resources
  • Complete annual data upload and submission to PortCF
  • Participate and coordinate staff training from material and sessions available for CF
  • Assist in obtaining Registry consents for patients new to the center
  • Complete and follow institution and IRB training requirements to participate in research
  • Assist PI, Center Director, or Research Coordinator as determined within individual centers as follows: consenting subjects, regulations on record keeping of consent forms and study records/documents, continuing reviews and study modifications
  • Generate center or patient specific reports and teaching others to generate reports using report writer tool
  • Assist in obtaining, uploading and submission of Annual Surveys

Specialized Services for Nursing Care
Newborn Screening for CF
  • Oversee newborn screenings processes performed at the local CF Center, as applicable.
  • Communicate with state newborn screening staff and establish processes specific to CF newborn screening
  • Evaluate all abnormal newborn screenings referred to the CF Center
  • Ensure that all positive cystic fibrosis tests are identified and followed appropriately by the CF Center team
  • Assist in arranging sweat chloride testing for infants with abnormal screens
  • Coordinate with core nursing and scheduling to update clinic templates as needed to accommodate add-on visits for infants with abnormal newborn screen
  • Provide newborn screening support and education to families
  • Assist with ambiguous or CRMS cases
  • Follow up on missed appointments, as necessary

Sweat test and Standards
  • Assist physicians with evaluation and follow-up on sweat testing done at the local CF Center
  • Arrange appointments as needed for testing and/or clinic appointments
  • Provide support and education information to parents/patients

DNA Mutation Analysis Results
  • Ensure that appropriate test forms for genetic testing are available
  • Assist in arranging for DNA testing
  • Assist physicians with evaluation and follow-up
  • Provide support and education as needed to patients/parents

Provide Education
  • Responsible for the oversight of CF education and the promotion of CF awareness within the center and community.
  • Serve as a resource for current, evidence-based information and best practices regarding CF and CF Care
  • May attend NACFC annually
  • Maintain a 'library' of the various CF educational materials provided by the CFF and by various pharmaceutical companies
  • Provide patient/family education, in-service lectures for staff, to include medical professionals, ancillary staff and community organizations, involved in care of the CF patient population in the community and center
  • Develop lectures or other format to create awareness of CF
  • Educate staff, patients and families on CF practice guidelines and statements
  • Mentor the CF team and assist individual team members with professional development according to specialty, include other sub-specialties such as GI, endocrinology, radiology and inpatient staff
  • Maintain involvement with local, regional and national CF education programs and meetings
  • Provide education to patients and their families on CF, CF Care and assist with facilitating self -management of their disease
  • Organize education opportunities for specific team members, e.g. airway clearance techniques by a respiratory therapist, feeding behavior issues with a dietician, etc.
  • Develop and implement a Quality Improvement plan to improve center care processes and health outcomes
  • Involvement with Family Advisory Board; assist advisory board with planning and execution of Family Education Day
  • Collaborate with local CFF Chapter to assist or participate in educational or fundraiser events and to disseminate information, if requested from national and local CFF chapters

Provide Leadership and Oversight for Specialized Services or Populations
Outpatient
  • Respond to patient/family messages or phone calls, forward information to necessary parties if needed to determine the appropriate response
  • Oversee and coordinate care provided at the CF Outpatient Clinic
  • Work with CF Team members to assure that all CF Foundation Guidelines for Outpatient Care are followed
  • Assure patients are seen at appropriate intervals per CFF recommendations
  • Work with CF team to ensure CF patients are reviewed at least annually for CF practice guidelines
  • Hold regular team meetings to review individual patient care and appropriate treatments per center criteria
  • Implement and monitor infection control practices
  • Assist as needed to ensure that education and program assistance materials are available and up to date
  • Assist with development of telephone triage process to ensure calls returned by appropriate staff in a timely manner (sick calls, refills, equipment issues, lab results, etc)

Education Requirements
  • A Baccalaureate degree in Nursing is required.

Experience Requirements
  • 1-3 years of nursing experience is required.
  • Demonstrated experience with pediatric nursing care.
  • Current license to practice nursing in Iowa is required.
  • Excellent written and verbal communication and interpersonal skills are required.

Highly Desired Qualifications
  • Experience with care coordination for patients across the spectrum of different specialties.
  • Demonstrated expertise in cystic fibrosis care is highly desired.
  • Ability to work independently as well as within a team.
  • Demonstrated experience in providing education to multiple stakeholders.
