Job description

The Licensed Practical Nurse is responsible for providing general nursing care to patients, under the direction of a Registered Nurse and in accordance with physician's orders and established standards and protocols. This position must integrate company values into daily practice.

Essential Functions:

  • Provides skilled nursing care to patients as ordered by the physician and under the supervision of a Registered Nurse.
  • Implements orders for medications and treatments as ordered by the physician and in accordance with nursing care plan as written by the RN.
  • Provides information to the Registered Nurse to assist in the development of an individualized plan of care for each patient.
  • Participates in evaluation of the effectiveness of interventions in relation to patient responses and seeks guidance in redirection when necessary.
  • Reports to supervisor on patient care rendered, problems concerning patients' needs, and resolution to those needs.
  • Helps identify patient problems and potential risks and contributes to the development of interventions.
  • Assists in identification of patient's learning needs and reinforces self-care and other techniques.
  • Appropriately designates applicable charges to individual patients using the hospital's established system.
  • May be required to work during inclement weather and other staffing emergencies.
  • Provides an environment conducive to safety for patients, visitors, and staff. Assesses the risks for safety and implements appropriate precautions. Complies with appropriate and approved safety and Infection Prevention standards.
  • Maintains regular attendance in keeping with assigned work schedule.
  • Performs other duties as assigned to support overall effectiveness of the organization.

Minimum Job Requirements


Minimum Education & Experience:

  • Two years' experience in inpatient hospital setting, inpatient rehabilitation experience preferred.


Required Licenses, Certifications, and/or Documentation:

  • Current state license as a Licensed Practical Nurse required.
  • Current BLS certification required.


Required Knowledge, Skills, and Abilities:

  • Knowledge of and adherence to current nursing theory and practice and infection prevention standards of practice.
  • Knowledge of accreditation standards to ensure adherence to all standards set forth by state and accrediting agencies of TJC and CMS.
  • Knowledge of clinical operations and procedures, and utilizes proper policies, procedures, and guidelines in caring for patients.
  • Demonstrates general computer skills including data entry, word processing, email, and records management.

Physical Requirements Over the Course of a Shift:

  • Possible exposure to bodily fluids.
  • Visual acuity required for patient assessment and documentation of care.
