REPORTS TO: Director of Recuperative Care

LOCATION: Los Angeles-Pico Union

CLASSIFICATION: Exempt, Full Time

SALARY: $60,000.00 - $70,000.00

  • Work Shift: Tuesday-Saturday, 8:30AM-5:00PM

Social Service Manager

The Social Services Manager provides oversight within the social services department and liaises with hospital social workers and/other external case manager representatives when necessary. Social Services Manager ensures maintenance of quality of care while providing overall leadership in social services disciplines at their respective recuperative care sites. In this role, they support organizational goals; apply professional experience and independent judgment to complex tasks; perform a wide variety of managerial and administrative functions to assess needs and ensure program objectives are met; serve as a community and organizational resource on issues of healthcare and homelessness. They are responsible for the management of all social services needs for recuperative care guests. Provide all necessary staff support and guidance to interns and Recuperative Care Social Service Coordinators and associates to ensure project objectives are achieved. Ability to shape program direction through strategic networking, collaboration, and internal and external relationships.


Responsibilities and Initiatives

To help NHF meet its growth goals, the Social Services Manager will:

• Provide direction, leadership and participate with providing information respective to social services department and program site grant funding activities with Director’s acknowledgement and guidance.

• Maintain quality guest care by completing quality assurance checks of guest files and sites.

• Ability to maintain caseload of 25 and above, when required, to meet the needs of the facility.

• Must be able to conduct and/or assign social services staff to complete home visits and provide necessary case management to ensure housing placement stability for guests who have been discharged to permanent residence within the Rapid Re-Housing (RRH) program.

• Ability to conduct house/group advisory board meetings for guests to address concerns or feedback.

• Ability to enter HMIS documentation and/or support social services staff in doing so and/or adhere to LAHSA standards, as applicable.

• Facilitate and provide on-going case review, case conferencing, and ensure outcome measures are meeting strategic goals for social services department.

• Audit and maintain accurate information contained in proprietary CRM database.

• Lead efforts to establish strategic partnerships with community resources that will benefit guest needs during their stay in the program and upon exit.

• Provide support as Social Services liaison between the Recuperative Care program and hospital social workers and/other external case manager as needed to deepen partnerships and resolve any issues.

• Develop programs, activities, or recreation that will provide support or resources for social issues (HIV, mental health, sobriety etc.) to improve program quality of care.

• Facilitate and/or coordinate relevant support groups at the sites with the guests that are present at least once a week.

• Provide direction and ensure compliance are maintained with appropriate guest interventions such as de-escalation, non-contact Crisis Prevention Intervention, motivational interviewing, etc.

• Provide continuous guidance to staff in the development of comprehensive discharge plans.

• Triage and address guest, staff, and programmatic concerns as needed.

• Ensure all guest notes and data collection is complete, timely, and accurate in the CRM database and/or e-charts by each social services coordinator or associate in accordance with the program’s database goals.

• Maintain, manage, and review department budget with Director.

• Ensure social services coordinators or associates are maintaining the bed board application or any bed reservations tool to determine census cap for operational planning i.e. guest meal planning etc.

• Conduct guest room searches, when necessary, and ensure harm reduction procedures for contraband are being adhered to.

• Complete weekly, bi-monthly, or monthly one-on-one supervision support and/or coaching and mentoring via Culture Amp and training to social service staff and student interns, if applicable.

• Collaborate with Director to provide training to program staff.

• Ensure the team has administrative hours to complete documentation.

• Attend all training and in-services as scheduled.

• Project a positive professional image and adheres to organizational dress code.

• Oversee and review social services coordinators or associates staffing schedules, timecard reviews for payroll, PTO requests and performance appraisals.

• Flexibility to adhere to schedule changes and on-call status in providing support to department and program staff as it relates to job functions and needs of the facility.

• Complete additional tasks and responsibilities as needed to meet the needs of the facility.

• Always follow NHF policies and procedures.

Requirements

• Bachelor’s degree in social work or related field and minimum of 3 years case management and supervisory experience.

• Master’s degree in social work or related field and minimum three years of experience working with the homeless population, healthcare.

• Knowledge of the organization and healthcare and homeless issues; requires extensive expertise in subject areas of projects.

• Making decisions based upon the analysis of the subject, phase or issues involved in each assignment.

• Working with homeless individuals, mental health, and substance abuse preferred

• Able to handle complex crisis situations and thrive in a fast-paced environment.

• Proficiency in Microsoft Suite products including Word, Excel, Access and PowerPoint and skill in using databases.

• Reliable transportation.

• Bi-lingual Spanish Preferred.

• TB test required.

• Fingerprint live scan /background required.
