Job description

Registered Nurse for SoNE HEALTH Medical Group

The Registered Nurse for SoNE HEALTH Medical Group provides high-quality, patient-centered care in a team-based environment. The RN supports providers by performing clinical tasks, managing chronic conditions, coordinating care, and promoting health education and disease prevention for patients across the lifespan.

This position requires strong computer skills, attention to detail, familiarity with some medical terminology, and experience with health insurance.

This position is located in Enfield, CT and is assigned to our Pediatric population. Work schedule is Monday - Friday, 8am - 5pm.

Essential Functions
  • Provides clinical and administrative support for Pediatric providers within the care team model. This includes:
    • Serving as a liaison between providers and their patients
    • Coordinating the State of CT Vaccine Program on behalf of the practice.
    • Communicating abnormal lab results and treatment plan to patients as outlined by the provider and/or protocols
    • Providing patient education in areas such as chronic disease and medications
    • Completing medication reconciliation and new patient intakes
    • Working with other members of the care team to coordinate patient care, referrals, care management and hospital/ER follow up
    • Executing nursing care under direction of the provider
    • Navigating medication changes and prior authorizations due to insurance requirements under standing orders and the guidance of the provider
  • Manages the clinical phone line. Assists pharmacies, responds to patient requests, conducts triage and conveys messages to and from providers, clinics, specialists, and other care team members.
  • Directs the assessment of acutely ill patients who present to the clinic in person or via phone to ensure efficient workflow.
  • Collaborates with medical support staff and assists with prioritizing clinical tasks to ensure efficient work flow. Assists with training and competency verification of medical support staff.
  • Participation in Care Management and Chronic Disease Management efforts in collaboration with the Population Health Department.
  • Documents all activity appropriately in the Electronic Medical Record.

  • Performs other duties as assigned.

Qualifications and Competencies
  • Current Registered Nurse (RN) license in Connecticut.
  • Experience in pediatrics is strongly preferred
  • CPR Certification
  • Prior experience in primary care, family medicine, or outpatient clinic setting preferred
  • Strong clinical assessment, communication, and organizational skills
  • Proficiency with electronic health records (EHR) systems
