Home Health Registered Nurse (RN) Case Manager
Location: Lewisville, TX

Overview:
Our well-established home health agency is seeking a Home Health RN Case Manager to deliver high-quality, patient-centered care in the Lewisville community. Our role oversees a caseload of home health patients, serving as the central clinical point of contact and coordinating care with the interdisciplinary team to drive optimal outcomes and regulatory compliance.

Position Offers:

  • Schedule: Monday–Friday
  • Competitive salary: $89,000 (DOE)

Key Responsibilities:

  • Conduct comprehensive initial and ongoing patient assessments in the home
  • Develop, implement, and update individualized care plans based on clinical findings and physician orders
  • Coordinate patient care, including scheduling visits and communicating with physicians, therapists, and team members
  • Complete accurate OASIS assessments and maintain compliant documentation in the EHR
  • Manage and reconcile medications, including patient education on safe administration
  • Provide direct skilled nursing care, including wound care and chronic disease management
  • Educate patients and families on diagnoses, treatment plans, and safety strategies
  • Monitor patient progress, adjust care plans, and escalate concerns as needed

Qualifications:

  • Active, unrestricted RN license in Texas
  • Associate Degree in Nursing (ADN) required; BSN preferred
  • Experience in home health nursing with case management responsibility
  • Strong knowledge of OASIS documentation and Medicare home health regulations
  • Current BLS certification
  • Reliable transportation for travel to patient homes in the Lewisville area
