Job description

We are seeking a compassionate and independent PRN Home Health Registered Nurse (RN) to join our dedicated healthcare team. As a PRN Home Health RN, you will be responsible for providing high-quality and personalized nursing care to patients in the comfort of their homes. You will assess patients' health needs, develop care plans, administer medication, and educate patients and their families on managing their conditions. Your role will be crucial in helping patients maintain their independence and improve their overall quality of life.

RESPONSIBILITIES

Conduct thorough assessments of patients' health status and develop individualized care plans in consultation with the healthcare team.

Administer medications, treatments, and wound care as prescribed by the physician.

Monitor patients' vital signs and symptoms and report any changes or concerns to the physician promptly.

Educate patients and their families on proper healthcare strategies, disease prevention, and medication management.

Collaborate with other healthcare professionals, such as LVN's and therapists to provide holistic care to patients.

Coordinate and communicate effectively with the interdisciplinary team, physicians, and other healthcare providers.

Document all patient care activities accurately and timely in electronic health records.

Provide emotional support and encouragement to patients and their families.

Ensure compliance with all applicable laws, regulations, and agency policies such as OSHA and CDC guidelines for Universal Precautions in home health care.

QUALIFICATIONS & REQUIREMENTS

Graduate of an accredited school of nursing with a valid and current Registered Nurse (RN) Licensed by the Texas State Board of Nurse Examiners.

Preferred minimum of 1 years of clinical nursing experience, preferably in home health or a related field with a minimum of 2 hours of general, critical care, or emergency hospital experience.

Strong clinical assessment and critical thinking skills.

Skills and knowledge with infusion therapy and wound care is a bonus but not required.

Excellent communication and interpersonal skills.

Ability to work independently and manage time effectively.

Proficient in using electronic health records and other healthcare software.

Compassionate, empathetic, and patient-centered approach to care.

Reliable transportation and valid driver's license.

Basic Life Support (BLS) certification.
