Job description

*** THIS POSITION MAY BE ELIGIBLE FOR UP TO A $15,000 SIGN-ON BONUS *** We have a fleet vehicle program available that supports this clinical role!
As a Hospital At Home (MACU- Mobile Acute Care Unit) RN, you will deliver direct clinical care to patients in their homes, engaging in multiple nursing visits and utilizing telehealth technologies. Collaborating with a diverse team of healthcare professionals, you will foster effective communication and coordination of care. This role allows you to broaden your skill set in complex care, including high-tech IV procedures, and the use of advanced medical equipment.
Performance Responsibilities and Standards
  1. Embrace the Unconventional: As a Hospital At Home (MACU- Mobile Acute Care Unit) RN, you will step into a non-traditional healthcare environment where you will provide direct hospital-level clinical care to patients in their private homes. Your role will encompass:
    • Engaging in multiple nursing visits per day, using remote patient monitoring and virtual visits to deliver continuous care and support.
    • Collaborating with medical providers at the hospital, effectively communicating changes in patient condition, routine clinical updates, and progress towards goals, leveraging your strong clinical judgment and ability to advocate for patients.
    • Administering medications and providing complex comprehensive care as ordered, embracing the autonomy and flexibility this setting offers.
  2. Explore a Multidimensional Approach: Working alongside a diverse team of healthcare professionals, including Advanced Practice Providers (APPs), physicians, and support staff, you will:
    • Foster direct and indirect communication with hospital personnel, creating synergistic relationships and ensuring seamless coordination of care.
    • Document patient care accurately and adhere to Quality Standards, embracing the opportunity to broaden your understanding of comprehensive care delivery.
    • Share your unique perspective and learn from other clinicians, contributing to an environment of collaborative learning and growth.
  3. Expand Your Horizons in Complex Care: This position fosters professional growth as you expand your skill set and expertise in providing quality complex care. You will:
    • Strengthen your proficiency in areas such as assessment, high-tech IV procedures, and the utilization of advanced medical equipment.
    • Engage in tasks involving catheter insertion and care, drainage tube management, wound vacs, complex wound/burn dressings, oxygen administration, and the use of durable medical equipment and telemonitoring devices.
  4. Fulfill any additional responsibilities and tasks as assigned, contributing to the overall success and excellence of our mobile acute care program.
  5. Other duties as assigned.

Requirements
  1. Nursing: License and current registration as a Registered Professional Nurse in New York State. Education: BSN preferred; Associates Degree required.
  2. Experience as a clinician in home and community-based services is highly valued.
  3. Ability to provide exceptional care to individuals in private homes, fostering a comforting and healing environment.
  4. Excellent written and verbal communication skills, enabling you to manage stressful situations and resolve conflicts effectively.
  5. Proficiency in utilizing technology and computer applications, including Microsoft Office or other word processing, spreadsheet, and database management tools.
  6. Strong organizational skills with the ability to prioritize tasks and manage multiple responsibilities effectively.
  7. Resourcefulness in problem-solving, coupled with the ability to motivate and educate others.
  8. Ability to work independently, multiple assignments, prioritize workload.
  9. Computer literate and working knowledge of basic computer skills and Microsoft Office programs.
  10. Strong communication skills person to person and by phone.
  11. Ability to learn organizations software system to perform job duties.

Physical Requirements
  • Ability to sit or stand for long periods during work day
  • Ability to perform tasks involving physical activity (including but not limited to): Pushing/pulling on average 50 lbs. with a max of 150 lbs.; lifting/carrying on average 25 lbs. with a max of 50 lbs.; patient transfers, extensive bending, standing, kneeling, walking in various weather conditions and ability to go up and down stairs.
  • Ability to drive in ALL weather conditions (rain, sleet, snow, wind).
  • Visual/hearing ability sufficient to comprehend written or verbal communication
  • Ability to use telephone and communicate over the phone
  • Frequently uses a laptop computer to enter orders and obtain product information
  • Bloodborne Pathogens Exposure Category I - Employee has tasks that routinely involve exposure or potential exposure to blood, body fluids or tissues.
