Location: CSU Health Services Center
Biweekly Hours: 80.00
Shift: Monday-Friday 8a-5p

Summary:

Serves as the primary liaison between patients of the clinic and The MetroHealth System while providing a high level of personal attention and concierge-style care to ensure patients can successfully navigate aspects of the healthcare system to fulfill their healthcare needs through strong communication and service. Delivers high-quality, high-touch primary care and care coordination in conformance with approved and recognized nursing standards of care, recognized nursing techniques, procedures, and administrative policy. Participates in health coaching, health fairs, coordinates biometric screenings, and other wellness activities. Assumes accountability for member satisfaction. Builds relationships with members, assists with care plans, and performs outreach. Upholds the mission, vision, values, and customer service standards of The MetroHealth System.
Qualifications:
Required:
Graduate of a state-approved School of Practical Nursing.
Currently licensed as License Practical Nurse by the State of Ohio through the Ohio Board of Nursing.
Current AHA Basic Life Support for the healthcare provider certification or obtains within 90 days of hire.
2 years of healthcare experience as an LPN, preferably in primary care, outpatient care, or care management.
EMR knowledge and experience.
Possesses a strong work ethic and ability to work independently and as a member of a multidisciplinary team.
Displays a passion and strong commitment to delivering excellent customer service, with a preference for doing so in a healthcare setting.
Knowledge of basic medical procedures and medical terminology.
Excellent written, verbal, and interpersonal communication skills, and able to use those with a diverse patient population.
Demonstrates strong analytical, data management and computer skills, including related to health-related data and data systems.
Proficiency with the Microsoft Office suite of products.
Detail-oriented and demonstrates strong leadership, organizational and time management skills.
Demonstrates solid judgement, flexibility, initiative, and problem-solving abilities.
Willing and able to work a flexible schedule, including some evenings or weekends.
Access to reliable transportation to travel between locations both within and outside The MetroHealth System based on operational needs.
Preferred:
Experience with the Epic electronic health record system.
Fluency in writing, speaking, and interpreting both English and another foreign language.
Physical Requirements: May sit, stand, stoop, bend, and ambulate intermittently during the day. May need to sit or stand for extended periods. See in the normal visual range with or without correction. Hear in the normal audio range with or without correction. Finger dexterity to operate office equipment required. Ability to lift 250-300 pounds with assistance. Ability to push/pull up to 500 pounds with assistance. Requires some exposure to communicable diseases or body fluids. Requires some exposure to chemicals and cleaning products. Ability to communicate in face-to-face, phone, email, and other communications. Ability to see computer monitor and departmental documents. Ability to read patient records, reports, etc.
