Job description

FLEXIBLE SCHEDULE 


Description of Duties and Responsibilities: 

  • Provide IIHS services that are family focused and consider the entire family's needs, and are delivered primarily in the family's home.
  • Provide team leaders with updates needed to individual service plans from consumers and families, as consumer needs change.
  • Coordinate a variety of services from a team approach that can be used for consistent planning for the consumer.
  • Monitor progress of the consumer, reassessing continued need and evaluate the quality of services received. 
  • Ensure consumers have viable crisis plans at all times and address consumer’s crisis immediately.
  • Ensure that all required information is obtained as required and routine documentation is recorded appropriately and within specified timeframes.
  • Transition consumers to more appropriate services when needed.
  • Assist consumers and families in locating and applying for appropriate services and resources. 
  • Ensure face-to-face contact with the consumer and monitor the efficacy of all services the consumer receives, increasing contacts as consumer acuity or program procedure may require. 
  • Report and document critical incident reports from consumer and family and ensure consumer safety in the event of incidents. 
  • Locate and coordinate sources of help from within the family and community so that the individual receives available natural and community supports.
  • Educate consumer, guardian and family on rights and responsibilities.
  • Hold regularly scheduled monthly team meetings to coordinate the person-centered plan, assuring that all relevant agencies and community members are present.
  • Provide parenting skills training and help the family build skills to increase their ability to deal with the youth’s mental health diagnosis.
  • Assist consumers in preventing, overcoming, and managing functional deficits in school, home and community.
  • Assist the primary caregiver and family in acquiring skills needed to assist the consumer in all functional life domains.
  • Provide documentation as required by Carolina’s Creative Counseling, PA to include but not limited to service notes.
  • Participate in all agency required meetings or committees.
  • Maintain current training requirements and personnel record requirements.
  • Perform miscellaneous job-related duties as assigned by supervisor and/or Management Team.


Work Schedule:

Hours vary based upon agency and consumer’s needs.


Supervision provided to Staff:

The supervisor for this position is the Intensive In-Home Services team leader who is a Licensed or Provisionally Licensed Social Worker or Practicing Counselor.


Required Education and Trainings: 

A minimum of a Bachelor's Degree is required, meets Qualified Professional Status as defined by 10A NCAC 27G.0104 rule, and has at least one year documented experience with the population served. Staff will complete 20 hours of Intensive In-Home Services training within the first 90 days of employment. 


Additional Requirements:

Staff have passed a nationwide criminal background and driving record check Staff possess a valid driver’s license, reliable transportation, automobile insurance, and current training.
