About the Role:

Family Care of Hartford Corp is looking for a passionate and dedicated Family Nurse Practitioner to join our growing healthcare team in Hartford, CT. This is an exciting opportunity to make a meaningful impact in the lives of patients across all ages while working in a collaborative and supportive clinical environment.

Responsibilities:

Provide comprehensive primary care services to patients of all ages, from pediatrics to geriatrics

Conduct thorough patient assessments, physical exams, and health screenings

Diagnose and treat acute and chronic illnesses, injuries, and conditions

Develop and manage individualized patient care plans and treatment protocols

Prescribe medications and therapies in accordance with state regulations and clinical guidelines

Educate patients and families on preventive care, wellness, and disease management

Maintain accurate and up-to-date electronic health records (EHR) documentation

Collaborate with physicians, specialists, and care teams to ensure continuity of care

Requirements:

Active Family Nurse Practitioner (FNP) certification from AANP or ANCC

Current Connecticut Advanced Practice Registered Nurse (APRN) license

Master's or Doctoral degree in Nursing (MSN or DNP) from an accredited program

DEA license and prescriptive authority in the state of Connecticut

3-5 years Experience in primary care, family medicine, or outpatient clinical setting preferred
