Position Overview

As a Health & Wellness Coach, you will provide prescribed medical treatment and personal care services to patients with diseases and injuries seeking treatment in the clinic, as directed by physician or mid-level provider. You will collaborate with the core PACT Team (Primary Care Provider, RN and Medical Assistant) and expanded PACT Team including family/caregiver, VA, and community-based services involved in developing the patient care plan.?



Core Responsibilities

  • Support clients in mobilizing internal strengths and external resources and develops self-management strategies for making sustainable, healthy lifestyles behavior changes and more effectively managing chronic disease.
  • Provide expert guidance and may offer resources from nationally recognized and approved authorities to contribute to patients’ overall wellness.
  • Coaches and assist patients by using insight, personal strengths and resources, goal setting, action steps and accountability toward healthy lifestyle change.
  • Assist patients with weight management, smoking cessation, addressing problematic areas in the eight components of proactive health and wellbeing.
  • Provides health coaching for patients to reduce or eliminate high-risk behaviors while increasing healthy ones.
  • Provides care through telephone clinics or secure messaging between face-to-face appointments.
  • Coordinates care with the patients’ treatment providers, both primary and specialty care, to ensure identified goals are consistent with best practices for treatment and management of chronic illness.
  • Actively guides patients in the exploration of core elements of personal health planning through individual and group-based programs.
  • Meets with external stakeholders to provide information about the Whole Health Group and individual programs and access to patients.



Requirements:
  • Must hold National Board Health Wellness Coach (NBHWC) certification or be eligible to sit for the exam by having the following:
  • Complete an NBHWC approved training program.
  • Complete 50 health and wellness coaching sessions; and
  • Have an associate degree or higher or 4,000 hours work experience (any field).
  • Ability to emphasis and use judgement on health enhancements/strengths, rather than disorders/weaknesses.
  • Adaptive learner and having the willingness to acquire knowledge on new technologies to solve problems.
  • Excellent problem solving and analytical skills.
  • Ability to work independently, self-directed, and solutions oriented.
  • Possesses sound organizational and planning skills with a solid mindfulness.


Final compensation will be based on relevant factors including work experience, skills, certifications, and internal equity.
