Community-Based Therapist - PT

Location: Port Saint Lucie, FL 34986

Job Summary

We are seeking a compassionate and skilled Community-Based Therapist to join our team in Port Saint Lucie, FL. The ideal candidate will provide therapeutic services to individuals and families within the community and homes, focusing on improving mental health and overall well-being through evidence-based practices.

Key Responsibilities
  • Provide individual, family, and group therapy sessions in community settings.
  • Conduct comprehensive assessments to identify client needs and develop personalized treatment plans.
  • Collaborate with community organizations, schools, and healthcare providers to coordinate care and support services.
  • Maintain accurate and timely documentation of client progress and treatment outcomes.
  • Utilize culturally sensitive approaches to engage diverse populations.
  • Participate in case conferences and multidisciplinary team meetings.
  • Promote mental health awareness and education within the community.
Qualifications
  • Masterโ€™s degree in Social Work, Psychology, Counseling, or related field.
  • Valid licensure as a therapist or Registered Intern (LCSW, LMHC, LPC, or equivalent) in the state of Florida.
  • Experience providing community-based therapeutic services preferred.

Essential Functions Statement(s)
โ€ข Conduct initial assessments of client needs and eligibility for services.
โ€ข Develop individualized treatment plans which fully addresses the client's needs, and
monitors treatment progress.
โ€ข Provide on-going evaluation, assessment, and direct clinical services to assigned
clients, using appropriate therapeutic techniques.
โ€ข Coordinate with schools, community agencies, and other service providers to ensure
provision of effective and appropriate services.
โ€ข Participate in court proceedings, clinical staffings, and educations meetings as
needed
โ€ข Coordinate and/or provides crisis intervention as needed
โ€ข Complete all required clinical documentation regarding course of treatment within
designated time frame. Ensures documentation meets quality and compliance
requirements.
โ€ข Consistently updates client files to ensure compliance Medicaid, payer, and company
guidelines.
โ€ข Consistently conducts a minimum of billable hours per week as established for
position (30 billable hours for full-time status and 15 hours for part-time status).
WORK ENVIRONMENT
This job operates in a professional office environment in a controlled atmosphere building.
This job also works clients homes and within the community, where the atmosphere is not
controlled by the employer. This position will also spend time traveling in their personal
vehicle.
