LPN / RN - Memory CareAnam Care & Anam Glen | Rockford, IllinoisShifts: Full-Time | 7:00 AM - 2:00 PM and 4:00 PM -8:00 PM | Every Other Weekend & Occasional HolidaysJoin a team that makes a meaningful difference every day.Anam Care and Anam Glen, a trusted assisted living memory care community in Rockford, is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our growing team. In this role, you will work closely with residents living with Alzheimer's disease and other forms of dementia, helping provide a safe, supportive, and dignified environment.We are looking for someone who is caring, dependable, and passionate about memory care. If you enjoy building relationships with residents and being part of a supportive team, we'd love to hear from you.ResponsibilitiesPrimary responsibilities include, but are not limited to:Administer medications and monitor resident responsesConduct nursing assessments and monitor changes in resident conditionComplete accurate and timely daily charting and documentationSupervise and support caregiving staffAssist with and support the community's memory care programmingCommunicate effectively with residents, families, and team membersRespond appropriately to medical emergencies or disaster situationsEnsure residents receive high-quality, compassionate careQualificationsActive RN or LPN license in the State of Illinois1+ year of experience working with Alzheimer's or dementia patients preferredStrong communication and teamwork skillsAbility to multitask in a fast-paced care environmentCompassionate, patient-centered approach to careCPR certification preferred (required after hire)ScheduleFull-time7:00 AM - 2:00 PM and 4:00 PM - 8:00 PM shiftsEvery other weekend requiredOccasional holidaysBenefitsWe value our employees and offer a competitive benefits package, including:Dental InsuranceVision InsuranceLife InsuranceVoluntary Life InsuranceFlexible Spending Accounts (Medical Flex Benefits)Dependent Care Flexible Spending Account
