Job description

Compensation 

Case Manager will receive 50% of what is collected from the case management services they provide The Commitment House clients. The monthly payment will be paid on the first Friday of the month. 

Responsibilities of Case Manager 

Case Managers will complete all required documentation: Prior Authorization, Assurance Form, Needs Assessment, Treatment Plan, 4 monthly contacts (2 direct, 2 indirect), and Monthly Summary. 

  • Maintaining accurate, timely, and professional records for all assigned clients.
    • Documentation will be on a 30-day cycle, ending on the 30th of the month and must be submitted by the 5th of the following month.
  • Managing no more than 30 active clients without prior approval.
  • Complying with all applicable laws, ethical standards, and The Commitment House policies.
  • Obtain and Maintain Trainings (i.e. TCM, SUD, SMI/Co-Occurring)
  • Obtain and Maintain required professional certifications, Supervision, CEU’s and liability insurance
    • (General liability limits of $1mm per occurrence, $3mm aggregate, work comp limits of $1mm).

Schedule & Caseload 

Case Managers will set their own schedule, provided all requirements are met.  

  • Case Managers will check in once a day via email, phone and/or physical presence.
  • Schedules shall be set in weekly form and submitted no later than the 1st of the month in collaboration with the program Manager.
  • Services are billable after 12:00pm Monday-Friday, and any time at the weekend. 

Case Managers are not permitted to provide services at any other agency while contracted with the Commitment House.  

Independent Contractor (1099) 

The Case Manager is a 1099 independent contractor, not an employee of The Commitment House. The Case Manager is responsible for their own: Federal, state, and local taxes, including self-employment taxes, Health, liability, and professional insurance, certifications and CEU’s. Case Managers are not eligible for employee benefits.
