Job description

S.P.A.R.C. (Sobriety, Peace, Awareness, Recovery, Center) is seeking a qualified Targeted Case Manager to provide professional case management services to individuals in need of behavioral health support.
Responsibilities:
  • Conduct assessments and develop individualized service plans.
  • Provide ongoing case management, advocacy, and linkage to community resources.
  • Coordinate care with providers, agencies, and natural supports.
  • Monitor client progress and adjust service plans as needed.
  • Maintain accurate documentation in compliance with state and agency standards.
  • Work in person from Russell Springs. Hours are 8 am to 4 pm, Monday through Friday.

Qualifications:
  • Bachelor’s degree in human services (social work, psychology, counseling, or related field).
  • A year of work experience after receiving college degree.
  • Strong organizational, communication, and interpersonal skills.
  • Ability to work both independently and as part of a team.
