Job description

The Long-Term Acute Care Registered Nurse provides planning and delivery of direct and indirect patient care to medically complex patients in long-term acute settings. 

Essential Duties and Responsibilities: 

  • Provides planning and delivery of direct and indirect patient care
  • Responsible for providing care to medically complex patients
  • Supervises care, treatment, and services of the patient(s)
  • Documents the patient’s plan of care using identified nursing diagnosis, expected patient outcomes and selected nursing interventions
  • Maintains confidentiality at all times with patients, medical staff and co-workers
  • Provides a safe, comfortable and therapeutic patient environment
  • Adheres to and supports the policies/procedures/goals/objectives of the hospital in an attempt to provide quality patient care 
  • Revises plan of care according to evaluation, changes in plan of care and nursing interventions
  • Performs interventions according to identified priorities, plan of care, and facility policies and patient care outcome standards
  • Identifies and prioritizes actions based upon patient care requirements

Minimum Requirements: 

  • Current RN license for the state in which the nurse practices in
  • One (1) year nursing experience in a long-term acute care facility preferred
  • Experience in acute rehabilitation for vents, wound care, cardiac, and stable post trauma patients
  • Complies with all relevant professional standards of practice 
  • Participation and completion of Amergis' Competency program when applicable
  • Current CPR if applicable
  • TB questionnaire, PPD or chest x-ray if applicable 
  • Current Health certificate (per contract or state regulation) 
  • Must meet all federal, state and local requirements 
  • Successful completion of new hire training as applicable to job site 
  • Understand patient confidentiality and HIPAA requirements  
  • Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required  
  • Computer proficiency required 
  • Must be at least 18 years of age
