    Job description

    Full Time Home Health Physical Therapist

    Full Time Home Health Physical Therapist provides patient-centered care in patients' homes, performing evaluations, developing treatment plans, and documenting progress. Works Monday to Friday, 8:00 AM โ€“ 5:00 PM, with no weekend duties, and supports patients in the Springfield/Branson area.

    Key Responsibilities:
    • Perform comprehensive patient evaluations and develop individualized treatment plans
    • Conduct start-of-care assessments and complete OASIS documentation accurately
    • Provide routine home-health therapy visits focused on improving strength, balance, mobility, and independence
    • Complete patient recertifications and reassessments as required
    • Educate patients and caregivers on therapeutic exercises, mobility strategies, and home safety
    • Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams
    • Monitor patient progress and modify treatment plans to maximize outcomes
    • Maintain timely and accurate clinical documentation using Matrix EMR
    • Deliver compassionate, patient-centered care in a home-based setting
    Qualifications:
    • Doctorate or Master's degree in Physical Therapy from an accredited program
    • Active Missouri Physical Therapist license required
    • Strong communication, organization, and time management skills
    • Ability to work independently and manage a daily patient schedule
    Preferred Skills:
    • Current CPR/BLS certification
    • Experience with OASIS documentation and Matrix EMR
