Job description

Home Health Registered Nurse

Full-time registered nurse provides home health admissions and start-of-care visits in the Kansas City metro area. The RN conducts patient assessments, documents OASIS data, and collaborates with interdisciplinary teams to develop individualized care plans. This role supports patient outcomes and ensures regulatory compliance.

Key Responsibilities:
  • Perform home health admissions and start-of-care visits for new patients
  • Conduct comprehensive patient assessments in the home setting
  • Complete and document OASIS assessments accurately
  • Utilize Matrix EMR for patient documentation and care planning
  • Collaborate with interdisciplinary care teams to develop individualized care plans
  • Educate patients and families on treatment plans, medications, and home health services
  • Ensure all documentation meets state, federal, and agency compliance requirements
  • Coordinate care with physicians, therapists, and other healthcare professionals
  • Monitor patient conditions and communicate changes to appropriate clinical staff
  • Travel between service areas as needed, primarily in Independence with occasional coverage in Overland Park
Qualifications:
  • Active RN license required
  • Compact RN license or active licenses in Kansas and Missouri
  • Graduate of accredited nursing program (ADN or BSN)
  • Minimum 1 year nursing experience required
  • Home health experience required
  • OASIS documentation experience required
  • Strong clinical assessment and communication skills
Preferred Skills:
  • Basic Life Support certification
  • Experience performing admissions or start-of-care visits
  • Experience using Matrix EMR
