    Job description

    Registered Nurse

    Full-time Registered Nurse focused on home health admissions and start-of-care visits in the Overland Park, KS area with occasional coverage in Missouri. The role requires comprehensive patient assessment, accurate OASIS documentation, and collaboration with interdisciplinary teams to develop individualized care plans.

    Key Responsibilities:
    • Perform home health admissions and start-of-care visits for new patients
    • Conduct comprehensive patient assessments in the home setting
    • Complete and document OASIS assessments accurately and in compliance with regulatory standards
    • Utilize Matrix EMR for patient documentation and care planning
    • Collaborate with interdisciplinary care teams to develop individualized care plans
    • Educate patients and families on treatment plans, medications, and home health services
    • Ensure all documentation meets state, federal, and agency compliance requirements
    • Coordinate care with physicians, therapists, and other healthcare professionals
    • Monitor patient conditions and communicate changes to appropriate clinical staff
    • Travel between service areas as needed, primarily in Independence with occasional coverage in Overland Park
    Qualifications:
    • Active RN license required
    • Must hold Compact RN license or active licenses in Kansas and Missouri
    • Graduate of accredited nursing program (ADN or BSN)
    • Strong knowledge of home health regulations and documentation standards
    • Minimum 1 year of nursing experience
    • Home health experience
    • OASIS documentation experience
    • Strong clinical assessment and communication skills
    Preferred Skills:
    • Current Basic Life Support certification preferred
    • Experience performing admissions or start-of-care visits strongly preferred
    • Experience using Matrix EMR preferred
