Our clients are supported by a team of Registered Nurses (RNs),Licensed Practical Nurses (LPNs), certified nurse practitioners, psychiatrists, mental health counselors, social workers, and case managers to ensure the best outcomes for those we serve. Work Environment

This position is based in a long-term care facility focused on providing mental health services to residents. The role requires flexibility, empathy, and a commitment to improving the quality of life for individuals with psychiatric needs.

Responsibilities may include:
  • Address the mental, behavioral and physical health needs of clients on a weekly/biweekly and/or monthly basis.
  • Complete in person nursing assessments as well as medication and chart review in long term care facilities (LTC).
  • Educate caregivers and clients on their prescribed psychiatric and physical health medications and monitor for possible side effects.
  • Care management duties including coordination of community services and correspondence with treatment team members, including physicians, pharmacies, employment programs.
  • Assist clients in building coping skills to address mood, stressors and improve communication and in creating annual goals as it relates to their psychiatric diagnosis.
  • Possibly assisting clinic and residential needs for assigned locations.
  • Help train and mentor peers.
  • May need to cover more than one facility depending on company needs
Requirements for this position include:
  • Graduate of an accredited nursing program.
  • LPN or RN license.
  • Experience in psychiatric, mental and behavioral health services.
  • Willingness to travel throughout assigned service area.
  • Preferably has experience working in LTC facilities as well as Behavioral Health.
  • CPR or Course can be provided during training
