Job description

We are seeking a board-certified Occupational Medicine Physician to provide employment and occupational health examinations for civilian employees supporting Grissom Air Reserve Base.

Services will be performed at the physician’s or partner clinic. The facility must be located within the area bounded by Grissom Air Reserve Base, South Bend, Fort Wayne, Lafayette, and Indianapolis, Indiana.

Responsibilities

  • Conduct pre-employment, periodic, fitness-for-duty, annual occupational, post-exposure, termination, and overseas employment examinations.
  • Evaluate firefighters, police/security personnel, respirator users, and other employees with position-specific medical requirements.
  • Review job requirements, workplace exposures, and medical documentation to determine medical qualification.
  • Complete and sign SF 178, SF 600, and applicable Air Force medical forms.
  • Perform or oversee physical exams, vision and hearing testing, ECGs, spirometry, laboratory testing, respiratory evaluations, radiology, and immunizations.
  • Conduct firefighter examinations in accordance with NFPA 1582 and applicable federal and Air Force standards.
  • Securely submit completed results and documentation within 48 hours.
  • Coordinate appointments and obtain approval before performing additional testing or follow-up services.

Qualifications

  • MD or DO degree.
  • Board certification in Occupational Medicine or Occupational Health.
  • Active, unrestricted license to practice medicine in Indiana.
  • Experience with occupational health, employment, or fitness-for-duty examinations.
  • Ability to meet HIPAA, credentialing, and 48-hour reporting requirements.
  • Access to a qualifying clinic with the equipment and resources needed to perform the required services.
  • Access to a CAOHC-certified technician for audiograms.

Experience with NFPA 1582 examinations, federal medical forms, law enforcement evaluations, or Department of Defense occupational health programs is preferred.

Appointments are generally scheduled Monday through Friday between 7:30 a.m. and 4:00 p.m. The anticipated volume is approximately 163 examination or testing services annually.
