Job description

Job Title: Licensed Master Social Worker (LMSW)

Location: New York, NY 10016
Schedule: Monday–Friday | 9:00 AM – 5:00 PM (1-hour lunch)
Hours: 35 hours/week
Contract Type: Contract
Compensation: $50–$60 gross weekly

Position Overview

We are seeking a Licensed Master Social Worker (LMSW) to support inpatient hospital populations. This role focuses on psychosocial assessment, care coordination, discharge planning, and patient advocacy while working collaboratively with an interdisciplinary healthcare team.

Key Responsibilities
  • Conduct psychosocial assessments for inpatient populations

  • Provide emotional support, counseling, and crisis intervention as needed

  • Participate in discharge planning and care coordination

  • Collaborate with physicians, nurses, case managers, and interdisciplinary teams

  • Connect patients and families with appropriate hospital and community resources

  • Document assessments, interventions, and care plans accurately in the electronic medical record (EMR)

  • Advocate for patient needs and ensure continuity of care

  • Maintain compliance with hospital policies, New York State regulations, and ethical standards

  • Participate in team meetings, case reviews, and quality improvement initiatives

Required Skills & Experience
  • Minimum 1 year of experience as a Licensed Master Social Worker

  • Hospital-based social work experience strongly preferred

  • Inpatient Medicine, Surgery (Med/Surg), or Rehabilitation experience preferred

Education
  • Master’s Degree in Social Work (MSW) – Required

Licensure & Certification
  • Active New York Licensed Master Social Worker (LMSW) license issued by NYSED

  • Primary Source Verification required
