Job description

Imagine360 is seeking a Contract Nurse Practitioner to join the team! miCare delivers on-site primary care clinics to remove both access and cost barriers for enrolled health plan members, offering greater opportunity for education and engagement. This Nurse Practitioner position is responsible for delivering office based primary care for males and females of all ages; includes chronic disease management with an emphasis on wellness along with acute episodic care.

Position Location: Clovis, CA

Responsibilities include but are not limited to:

General Duties

  • Utilizes knowledge and skills to care for patients under the conformance of current and recognized techniques, procedures and standards. Includes but not limited to:
    • physical examinations
    • orders, interprets and evaluates diagnostic tests to assess patients clinical problems and health care needs
    • records findings and formulates plan and prognosis
    • prescribes necessary prescriptions and other forms of treatment
    • refers appropriately to other fields of specialization
    • performs effective communication with miCare team and patient to ensure coordination of care.

Customer Service

  • Acts as a role model in demonstrating the core values in customer service delivery.
  • Provides timely and thorough follow up with, internal and external customers.
  • Appropriately escalates difficult issues up the chain of command.

Quality Assurance

  • Offers feedback and assistance in improving customer satisfaction.
  • Recognizes and alerts appropriate supervisor of trends within their scope of responsibility that fall outside of quality parameters.
  • Performs self quality monitoring in order to develop and execute plans to meet established goals.
  • Collaborates with others and cross-departmentally to improve or streamline procedures.
  • Provides feedback and develops new or improves current internal processes to optimize quality
  • Trains new employees as assigned

Required Experience/Education:

  • Degree from accredited school
  • Current unencumbered license to practice in the state of the Health Center
  • Minimum of three years clinical experience
  • Current BLS
  • Working knowledge of computers and software including but not limited to Microsoft Office products

Skills and Abilities:

  • Proficient typing skills
  • Demonstrated organizational skills, problem-solving, analytical skills and detail oriented
  • Strong written and oral communication skills
  • Demonstrated ability to work independently, prioritize workloads multi-task and manage priorities in order to meet deadlines
  • Ability to maintain the confidentiality of protected health information in compliance with HIPAA regulations

License and Certifications:

  • Must maintain an active, current, Nurse Practitioner license/certification in good standing, including all continuing education requirements as mandated by the California Board of Registered Nursing. Must maintain current Basic Life Support (BLS) certification. Additional licensure and/or certifications may be required to perform the essential functions of the position.

    Must maintain national board certification as a Nurse Practitioner appropriate to specialty and practice area, or obtain and maintain any additional certifications required by the organization or state regulations.
