Job Description
The Registered Nurse in the Case Management department is responsible for developing and implementing effective discharge plans and coordinating patient care while adhering to established protocols. This role is essential in ensuring high-quality healthcare delivery and smooth transitions for patients within the hospital setting.
Responsibilities
  • Conduct comprehensive assessments for case management.
  • Develop and implement individualized discharge plans.
  • Collaborate with multidisciplinary teams to coordinate patient care.
