Home Health RN Case Manager

Registered Nurse (RN) – Home Health Case Management

Provide Leadership. Coordinate Care. Improve Patient Outcomes.

Are you an experienced Registered Nurse with a passion for coordinating patient care and managing complex caseloads? We're seeking a compassionate Home Health RN Case Manager to oversee and coordinate individualized care plans for patients receiving skilled nursing services in their homes.

In this role, you'll serve as the primary clinical coordinator, ensuring patients receive the right care at the right time while collaborating closely with physicians, therapists, caregivers, and the interdisciplinary team. Strong case management experience is essential.

Responsibilities

  • Manage a caseload of home health patients from admission through discharge.
  • Perform comprehensive Start of Care (SOC), Recertification, Resumption of Care (ROC), and Discharge assessments.
  • Develop, implement, and regularly update individualized patient care plans.
  • Coordinate services with physicians, physical therapists, occupational therapists, speech therapists, social workers, and home health aides.
  • Monitor patient progress and adjust care plans to achieve the best clinical outcomes.
  • Conduct skilled nursing visits including medication management, wound care, disease management, and patient assessments as needed.
  • Identify changes in patient condition and communicate recommendations to physicians.
  • Educate patients and family members regarding medications, disease processes, safety, and self-management.
  • Ensure compliance with Medicare Conditions of Participation, OASIS documentation, agency policies, and regulatory requirements.
  • Complete accurate and timely electronic documentation.
  • Participate in interdisciplinary case conferences and quality improvement initiatives.

Qualifications

Required (Deal Breakers)

  • Current Registered Nurse (RN) license in good standing.
  • Previous Home Health RN Case Management experience.
  • Experience managing an independent patient caseload.
  • Experience completing OASIS Start of Care (SOC), Recertification (RECERT), Resumption of Care (ROC), and Discharge assessments.
  • Strong care coordination and interdisciplinary case management experience.
  • Excellent clinical assessment and critical thinking skills.
  • Ability to work independently while managing daily scheduling and documentation.
  • Valid driver's license and reliable transportation.

Preferred

  • BSN preferred.
  • Experience with electronic medical records (EMR).
  • Knowledge of Medicare home health regulations and documentation standards.
  • Wound care certification or chronic disease management experience is a plus.
