We are seeking an URGENT Contingent Center Registered Nurse (Float) to join our mission-driven team in San Francisco. In this vital, high-touch role, you will provide comprehensive clinical nursing care directly to elderly patients at our Day Health Center, covering an employeeโ€™s three-month leave of absence. Trading rapid-fire, episodic hospital triage for deep community advocacy, you will handle patient assessments, build preventative care blueprints, and serve as a central clinical consultant to an integrated, multidisciplinary team. Duties & ResponsibilitiesComprehensive Geriatric Care & Assessment
  • Intake & Routine Evaluations: Conduct thorough nursing assessments to evaluate the physical, mental, and psychosocial needs of frail elderly patients upon initial intake and during routine re-evaluations.

  • Care Plan Execution: Direct, implement, supervise, and continuously evaluate tailored nursing interventions for seniors directly within the Day Health Center.

  • Patient & Family Counseling: Empower patients, family caregivers, and geriatric aides by providing clear training, health education, and counseling regarding chronic disease management and prescribed care plans.

  • Clinical Consultation: Act as a specialized nursing consultant to the broader care team, collaborating closely to support on-site Staff Physicians and Nurse Practitioners.

  • Team Problem-Solving: Participate actively within a multidisciplinary team framework, offering critical clinical expertise during care planning, strategic benchmarking, and holistic problem-solving.

  • Fluid Floating Coverage: Serve as an adaptable float resource, stepping into clinical center coverage seamlessly to ensure absolute continuity of safe, compassionate care during temporary staffing shortages.

  • Other duties as assigned.

Required Qualifications
  • Licensure: Current, active, and unrestricted California Registered Nurse (RN) license.

  • Life Support: Active CPR / Heartsaver First Aid CPR AED certification.

  • Geriatric Experience: Minimum of one (1) year of professional nursing experience working directly with a frail or elderly population.

  • Communication Baseline: Fluent, strong written and verbal English communication skills.

  • Contract Commitment: Full availability to work a 3+ month contract assignment to cover an active employee leave of absence.

Desired Qualifications
  • Language Capabilities: Bilingual fluency in English and Cantonese is highly preferred to support our diverse local community.
