C.N.A./Hospice/Home Care - Home Health Aide Position Purpose:
Our hospice/home health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.
What You Will Do:
  • Follows care plan developed by RN.
  • Reports client changes in condition to RN, as described on care plan.
  • Assists clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.
  • Assists clients with household tasks including, planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.
  • Measures and records client's oral, rectal and axillary temperatures, pulse respiration and blood pressure as ordered.
  • Performs range of motion and other simple procedures as an extension of therapy services as assigned.
  • Assists clients in self-administration of medication. This procedure must be assigned by an RN and documented in the clinical record. The Home Health / Hospice Aide shall not administer medication of any kind
Minimum Qualifications:
  • High School diploma or GED required.
  • Completion of CNA 75 hr course.
  • Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.
  • Documentation of one year's employment as an aide in an institution, home care, or hospice setting.
  • Meets agency hiring criteria as defined in agency minimum standards (e.g. provision of professional references.)
  • Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with agency's client and their family.
  • Must accept responsibility for maintaining skills, learning and adhering to agencies policies and procedures on an on-going basis.
  • Ability to consistently demonstrate the Organizational Core Competencies and Organizational Code of Ethics, and adhere to the Compliance Program.
  • Must have current Driver's license and reliable transportation to and from work site.
