Summary of Position
The Connected Care Coach is responsible for proactively contacting identified members to drive behavior changes and positively impact clinical outcomes.
Essential Duties and Responsibilities
  • Conduct member outreach to drive behavior change and positively impact clinical outcomes (95%)
    • Actively engage with members via a variety of communication tools including telephone, email, text, virtual application, and coaching conversations to provide feedback and support, answer questions, share resources, and hold members accountable to their goals.
    • Utilize a coaching approach congruent with motivational Interviewing in order to empower members to be able to effectively self-manage their own chronic condition(s) and health.
    • Refer members for support and education for conditions as needed based on clinical care guidelines.
    • Maintain accurate and thorough case documentation to the standards set by Teladoc Health.
    • Complete administrative duties essential for smooth operation
  • Support corporate initiatives and special projects as needed (5%)

Qualifications Expected for Position
  • Bachelor's Degree in relevant discipline or MA/EMT/Paramedic certification
  • 2+ years experience in a healthcare delivery or related environment
  • Excellent written and verbal communication skills

  • Analytical and critical thinking skills which enable to work autonomously in a responsible, efficient manner
  • Possess excellent customer service and interpersonal skills to provide a consistently high-quality member experience.
  • Flexible and able to work in a fast-paced environment
  • Adaptable, innovative, and embraces teamwork and change.
  • Able to learn and effectively utilize multiple technology platforms including but not limited to MS word, Excel, PowerPoint, Outlook 365, Microsoft Teams, SharePoint, OneDrive, Salesforce, and Jira
  • Bilingual Spanish and English, preferred

The base salary range for this position is $24 - $26/hrIn addition to a base salary, this position is eligible for a performance bonus and benefits (subject to eligibility requirements) listed here: Teladoc Health Benefits 2026. Total compensation is based on several factors including, but not limited to, type of position, location, education level, work experience, and certifications. This information is applicable for all full-time positions.
During your first year of service, you will accrue 5.84 hours of PTO for each bi-weekly pay period. In addition, you will receive 80 hours of Paid Sick, Safe, and Caregiver Leave annually. This applies to full-time positions only. If you are applying for a part-time role, your recruiter can provide additional details.
