We are looking for an experience Case Manager LVN - Full-time. We offer FREE cell phones w/unlimited talk/text/data plans to all full time employees (restrictions apply).
The Case Management Department is responsible for promoting the efficient utilization of skilled services by ensuring that patients are admitted according to medical necessity. That services are provided to meet the assessed need of each patient in a prompt and efficient manner, and in accordance with patients insurance.
To facilitate that patients are discharged to the most appropriate environment with the necessary supportive services to meet their ongoing care needs.
That facility receives proper authorization of care and level of service.
