CHMC -RN Case Manager - Full Time-Day Shift - Req 9467
Pay Range: $37.63-$49.86*
We are seeking a highly organized and clinical RN Case Manager to oversee the care of complex patients. In this role, you will act as a central coordinator, negotiating and managing resources to ensure high-quality, cost-effective outcomes from admission through discharge.
Key Responsibilities
  • Care Coordination: Manage complex patient cases by collaborating with interdisciplinary teams both inside and outside the organization.
  • Strategic Planning: Merge clinical needs with financial processes, interpreting Medicare, Medicaid, and private insurance benefits to facilitate care.
  • Quality Improvement: Participate in performance evaluation processes to enhance patient care across the entire continuum.
  • Documentation & Compliance: Maintain accurate records, ensure patient confidentiality (HIPAA), and adhere to all regulatory and safety standards.
  • Advocacy: Use evidence-based best practices and disease management knowledge to guide treatment regimens and patient safety.

Qualifications & Requirements
  • Licensure: Current New York State RN License and registration.
  • Experience: Minimum of two (2) years of nursing experience.
