What you will do

We are seeking dedicated and experienced Community Access Support Workers to join our Explore Program team and provide exceptional support to clients within the community.

As a Support Worker, you will play a crucial role in helping individuals with disabilities achieve their goals, live independently, and participate fully in community life. This is a community-based role supporting clients out in the community rather than within SIL or residential homes.

  • Providing personalised support to clients in accordance with their individual support plans
  • Supporting clients to access community activities, appointments and social programs
  • Assisting clients to build independence and achieve their personal goals
  • Delivering support aligned with Behaviour Support Plans and NDIS Practice Standards
  • Supporting clients with varying support needs, including high intensity needs where required
  • Championing choice, dignity and cultural respect in all client interactions
  • Maintaining accurate and up-to-date documentation

About you

  • Hold minimum Certificate III in Individual Support or above
  • A passion for safeguarding human rights and working within a purpose-driven organisation
  • Hold or be willing to obtain a current NDIS Worker Screening Check and Working With Children Check
  • Current First Aid (or willingness to obtain)
  • Current Driver's Licence
  • A reliable vehicle and willingness to transport clients for community-based duties (mileage reimbursed)
  • Passport, Birth Certificate or Citizenship Certificate to validate Australian work rights
  • Willingness to complete a National & International Police Check (paid by Afford)
  • Ability to complete a Pre-Employment Medical/Functional Assessment (paid by Afford)
