Job description

) Social Worker
Sans Souci Rehabilitation and Nursing Center is seeking a talented Social Worker to join our team
Social Worker Responsibilities Include:
  • Assumes authority, responsibility, and accountability for the delivery of medical related social services in the facility. Collaborates and coordinates in an efficient and economic sound process with the interdisciplinary team
  • Identifying individual social and emotional needs
  • Developing and maintaining individualized care plans
  • Participating in the planning of the resident's admission, community re-entry or transfer to another facility
  • Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
  • Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
  • Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community

Social Worker Qualifications/Requirements:
  • BSW or MSW preferred, or Bachelors degree in related field
  • At least one year experience in Social Work field required
  • Long term care and/or sub-acute setting preferred
  • Ability to work flexible schedule
  • PCC experience preferred
  • Knowledge of MDS 3.0 preferred
