Community Relations Manager (Healthcare Outreach & Growth)Position Summary

Hamilton Homecare is seeking a motivated and relationship-focused Community Relations Manager to help build awareness, develop partnerships, and grow our presence throughout the Denver Metro area.

This role is responsible for developing meaningful relationships with healthcare professionals, community organizations, and referral partners while educating families and professionals about Hamilton Homecareโ€™s services.

The ideal candidate is a strong communicator who enjoys networking, building connections, representing a mission-driven organization, and creating opportunities that improve access to quality home care services.

Essential Duties & Responsibilities

Community Relations & Referral Development

Develop and maintain strong professional relationships with referral sources including hospitals, skilled nursing facilities, assisted living communities, physiciansโ€™ offices, care managers, senior organizations, and community partners.

Conduct regular outreach visits, presentations, meetings, and networking activities to introduce Hamilton Homecare services.

Identify new partnership opportunities and community resources that support seniors and their families.

Attend community events, senior expos, healthcare networking events, and educational opportunities.

Build trust and credibility within the community through consistent and professional communication.

Family Outreach & Client Growth

Respond to inquiries from prospective clients and families with professionalism, compassion, and urgency.

Educate families on home care options and how services can support safety, independence, and quality of life.

Assist families through the initial inquiry process and coordinate next steps with the appropriate team members.

Maintain consistent follow-up with prospective clients and referral partners.

Identify opportunities to improve client experience and strengthen community relationships.

Marketing & Brand Representation

Represent Hamilton Homecare as a trusted resource within the community.

Support outreach campaigns, educational initiatives, and community awareness efforts.

Collaborate with leadership on marketing strategies, events, and relationship-building activities.

Maintain a professional presence that reflects the values and mission of Hamilton Homecare.

Tracking & Reporting

Maintain accurate records of outreach activities, referral relationships, community contacts, and client inquiries.

Track relationship development activities and provide regular updates to leadership.

Maintain organized documentation within company systems.

Qualifications

High school diploma required; associate or bachelorโ€™s degree in healthcare, business, marketing, communications, social services, or related field preferred.

3+ years of experience in healthcare outreach, sales, relationship management, senior services, home care, healthcare staffing, or a related field preferred.

Experience developing referral relationships within the healthcare or senior care community strongly preferred.

Valid driverโ€™s license, clean driving record, and reliable transportation required.

Knowledge, Skills & Abilities

Strong interpersonal, communication, and presentation skills.

Ability to build genuine relationships and establish trust with professionals, families, and community members.

Self-motivated with the ability to work independently and manage priorities.

Comfortable with community networking, outreach activities, and meeting new people.

Working Conditions

Frequent travel throughout the Denver Metro area for meetings, outreach activities, and community events.
