Community Outreach Coordinator (Field-Based)

A Place Called Home LLC

Location: Woodbury, MN (Travel Required Throughout the Twin Cities Metro Area)
Position Type: Part-Time (30 Hours Per Week)
Compensation: $26.00 per hour
Schedule: Three days per week | Flexible Schedule

This is NOT a remote or office-based position.

This role requires approximately 80–90% of your time to be spent in the field, conducting face-to-face visits and building professional relationships throughout the Twin Cities metropolitan area.
You will regularly travel to hospitals, county agencies, treatment centers, behavioral health providers, housing organizations, community partners, and other referral sources.

About A Place Called Home LLC

A Place Called Home LLC is a growing Minnesota 245D Home and Community-Based Services (HCBS) provider dedicated to helping individuals with disabilities and behavioral health needs live safely and independently in their communities.
As we continue to grow, we’re seeking a motivated Community Outreach Coordinator to help us expand our network of referral partners and increase awareness of the high-quality services we provide.
Position Summary

The Community Outreach Coordinator serves as the public face of A Place Called Home LLC throughout the community.
Your primary responsibility is building relationships—not sitting behind a computer.

You’ll spend your days traveling throughout the Twin Cities introducing our organization to professionals who regularly work with individuals needing community-based services.
Success in this position depends on your ability to build trust, establish partnerships, and represent our organization with professionalism and enthusiasm.
What You’ll Be Doing

Your work will primarily take place outside of the office and throughout the community.
You’ll regularly visit:

  • Hospital Social Workers
  • County Case Managers
  • HealthPartners Care Coordinators
  • Behavioral Health Clinics
  • Mental Health Providers
  • Psychiatric Hospitals
  • Treatment Programs

Responsibilities include:
  • Conducting in-person outreach visits
  • Developing long-term referral partnerships
  • Educating professionals about our services
  • Following up with referral sources
  • Representing A Place Called Home at networking events
  • Maintaining outreach activity and referral records
  • Identifying new opportunities for community partnerships
  • Collaborating with leadership to increase referrals and community presence

  • Existing professional relationships within healthcare or human services are a plus
