Work Environment

This is a full-time position with 4 days onsite at UC Center for Women's Health and 1 remote day per week. The specific onsite days will be determined in coordination with Center for Women's Health and Cradle Cincinnati Leadership.

Position Summary

Cradle Cincinnati is dedicated to improving maternal and infant health outcomes and eliminating racial disparities in birth outcomes across Hamilton County. Through community-driven programs, healthcare partnerships, and quality improvement initiatives, Cradle Cincinnati works to ensure every birthing person and baby has the opportunity for a healthy pregnancy, birth, and first year of life.

The Queens Village Navigator is a community-embedded position employed by Cradle Cincinnati and stationed full-time within the University of Cincinnati Medical Center - Center for Women's Health. Serving exclusively Black birthing individuals, the Navigator bridges clinical prenatal care and culturally centered community support by integrating Queens Village programming into prenatal workflows, strengthening referral pathways, and connecting patients to wrap-around resources throughout pregnancy, birth, and the postpartum period.

Key responsibilities include:

Patient Engagement & Care Navigation

  • Actively connect with all eligible Black birthing individuals during prenatal and postpartum care visits

  • Provide support and connection to Queens Village programming by building relationships, maintaining continuous communication throughout pregnancy and the postpartum period

  • Connect patients to internal resources to address Social Determinant of Health needs.

  • Receive patient referrals via Epic and complete timely follow-up during onsite days

Clinic Integration & Team Collaboration

  • Participate in daily clinic huddles and interdisciplinary meetings to align patient flow and identify engagement opportunities.

  • Build relationships with physicians, nurses, social workers, residents, and clinic staff to normalize Queens Village as part of the care team.

  • Provide ongoing education and guidance to clinic staff on Queens Village programs and referral processes.

Documentation

  • Document all patient interactions and communications in the Electronic Health Record (EHR)

  • Collect appropriate data for Cradle Cincinnati reporting requirements

  • Document any community resource referrals; record relevant feedback from those resources in the patient EHR to support care team collaboration.

Queens Village Corner & Community Programming

  • Maintain and curate the Queens Village Corner in the main lobby, ensuring it is welcoming and stocked with up-to-date resource materials.

  • Promote and share information about Queens Village community events with all eligible patients.

  • Coordinate onsite Queens Village programming/activities in partnership with UC Center for Women's Health leadership

JOB QUALIFICATIONS

  • High school diploma or equivalent

  • 2+ years of work experience in a related job discipline

  • Experience working within clinical/healthcare settings

  • Familiarity with maternal and infant health, social determinants of health and health disparities

  • Preferred: Current Community Health Worker (CHW) certification in the state of Ohio

  • Preferred: Experience serving, collaborating with, and building relationships with diverse populations within urban communities.
