Community Health Nurse

Tapestry 360 Health

Lakeview Clinic

Summary: A leading member of the health center care team, the Tapestry 360 Health (Tap360) Community Health Nurse assists with direction of daily patient visits, providing assessment and triage, care coordination, and health education and information to patients and families in a culturally appropriate and relevant manner. The nurse works with the health center team to assure the maintenance of a safe patient care environment and safe transitions in care both within Tap360 and with other community agencies and contributes to quality assurance and other organizational activities as needed.

Essential Duties & Responsibilities:

  • Assists the health center manager, and provider of the day with direction and daily flow of patient visits. Triages walk-in visits and telephone calls, facilitates medication refills, and administers medications.
  • Assures safe patient care environment. Oversees infection control practices, and the supply and storage of medication
  • Collaborates with care team members to assist patients during transitions of care:
    • Provides necessary support for the coordination of referrals, including the access of necessary tests or other values and reports
    • Provides information and support to patients and families and works with the care-team to assure appropriate follow-up.
    • Provides Prenatal Care Coordination
      • Coordinates with prenatal providers and OB team to address urgent triage situations.
      • Participates in prenatal health education.
      • Participates in monthly OB team meetings.
  • Provides health information, promotes health and well-being for patients and families by assessing health literacy, providing meaningful health information, and supporting healthy behaviors and self-management of chronic illness.
  • Supports Medical Assistants by providing information, ongoing development and training.
  • Provides support to Medical Assistants at the School Based Health Centers, specifically:
    1. Ongoing training and serves as education resource for medication safety,
    2. Assisting managers with oversight of monthly medication safety rounds and emergency kit maintenance, if requested.
    3. Annual CLIA-WAIVED competencies.
  • Updates and inputs patient progress notes into the EHR according to organizational standard
  • Provides assistance to the practice manager, for the annual vaccine program.
  • Updates and inputs progress notes in patient charts in the EHR according to organizational standards.
  • Other duties as assigned.

Additional Responsibilities (dependent upon location and/or care team) :

  • Super user on electronic medical record
  • Group education
  • Nurse Visits including but not limited to: Administration of medications such as immunizations and long-acting injectables; Provision of health education/chronic disease management support; Reconciliation of medications; Treatment of sexually transmitted infections; Provision of emergency contraception.
  • Actively identifies community resources, fosters relationships with community agencies and participates in other community health activities.

  • Education and/or Experience:
    1. Active Illinois Registered Nurse License required.
    2. CPR certification required.
    3. Bachelor of Science in Nursing preferred.

    5. Bilingual ability in Spanish/English preferred.

Pay Transparency: The salary range for this position is $70,000 – $90,000 annually. Actual compensation will be determined based on factors including, but not limited to, relevant experience, qualifications, internal equity, and organizational needs.
