The Community Health Ambassador (CHA) builds trusted relationships with residents, promotes wellness, and supports access to health services. CHAs serve as liaisons between community members and health systems, helping to identify needs and connect people to appropriate resources.
Job Responsibilities
Community Engagement & Relationship Building
  • Build relationships with residents to support their health, wellness, and social needs.
  • Collaborate with local health providers, organizations, and community leaders.

Outreach & Education
  • Host wellness events, workshops, and health-related activities.
  • Provide outreach and education on public health topics, including chronic conditions, mental health, and substance use.

Data & Program Support
  • Assist with data collection, program evaluation, and gathering community feedback

Professional Development & Team Participation
  • Participate in team meetings, trainings, and ongoing professional development opportunities

Work Environment & Physical Demands
The Community Health Ambassador role operates in a dynamic, community-based environment that includes both indoor and outdoor settings, such as community centers, public housing sites, and partner agency locations. The position requires regular travel within the assigned community, frequent interaction with residents, and collaboration with various stakeholders. Physical demands may include standing or walking for extended periods, occasional lifting of materials up to 25 pounds, and facilitating outdoor wellness activities. The role also involves computer-based tasks, data entry, and virtual meetings. Flexibility in work hours, including evenings and weekends, may be required to meet community needs.

Salary Description
The hourly wage for the Community Health Ambassador position ranges from $22.00 to $26.00 per hour based on experience and qualifications. This pay rate is aligned with industry standards and salary benchmarking for similar roles within community health outreach programs. The salary range is also consistent with the available funding and grant approval for this position. Pay will be determined based on the candidate's experience, skills, and alignment with the position requirements.
Qualifications
  • Strong communication and interpersonal skills.
  • Interest in promoting and supporting community wellness.
  • Knowledge of HOPE SF communities or lived experience in a HOPE SF site (required).
  • Ability to work flexible hours, including some evenings and weekends.
  • Comfort working with diverse communities.
  • Basic computer skills (email, data entry, virtual meetings).

Additional Requirements:
  • This is a time-limited, two-year position.
  • CHAs must enroll in or work toward completing a Community Health Worker (CHW) Certificate Program or a comparable pathway during the position.
  • Continued employment depends on satisfactory progress in the CHW program or approved alternative.

Preferred Qualifications
  • Experience in peer support, outreach, case management, or health promotion.
  • Bilingual or multilingual skills.
  • Familiarity with trauma-informed care, harm reduction, or public health approaches.
